Lewisohn's book was based soley on listening to session tapes, so he'd
have no first-hand knowlege of instruments used unless something was
listed on the EMI log or mentioned on tape. Of course, sometimes it's
obvious. Still, it's beyond the scope of his book.
The book "Beatle Gear" is a better source, though the author doesn't
attempt to identify every song each instrument was used on.
